1. Variation to candidature enrolment forms
All required forms concerning variations to candidature enrolment i.e. leave of absence, change of supervisor, change of thesis title, application for overseas research etc. can be downloaded from the following website.
2. Thesis submission forms
All thesis submission related forms, i.e. appointment of examiners, author’s consent form, etc. can be downloaded from this website.
3. Finance and Travel Forms
The Faculty of Science provides funding to support research-related activities of its Higher Degree Research candidates.
All research-related travel must comply with the University's travel policies. To access the Faculty of Science travel pack and per diem information, Faculty of Science Fax Order form etc, please go to this website.
4. Fieldwork Forms
Before you undertake any fieldwork, including overseas data collection, please seek advice from your academic supervisor and/or Department's Fieldwork Manager. You are required to fill out the appropriate notification and risk assessment forms.
5. Notification of Approved Offsite Research
HDR candidates must complete this OSR form to obtain Faculty approval to conduct research overseas/offsite (*Away from Macquarie University, North Ryde).
Please submit this form to Faculty’s HDR Team before commencing the overseas/offsite research and attach evidence that all requirements such as ethics approval, medical insurance and travel approval including insurance have been met for conducting the offsite research.
Once approved, a copy of the form will be given to the candidate as proof of approval of the overseas/offsite research. The original copy will be submitted to HDRO to be kept in the candidate’s file.
